URGENT- FOR ALL SICK NOTES/FIT NOTES
You no longer need to contact the surgery by telephone to request a sick/fit note from our surgery.
Please either
1) download and print this form (see below) and complete. please drop off the printed copy in person at the surgery
2) OR alternatively download the form from below and fill it in electronically via your device and simply email us the completed form to the following email address:
Free flu jabs are still available for those who haven’t had them!
Anyone over the age of 50 who has not had their free flu vaccine please contact the surgery to obtain a vaccine – late night appointments are available please ring us also if you have any doubts.
